What will you need?
Before your first session with our counsellor, we will need the following details:
- A referral from your SIRA nominated treating doctor
- Your SIRA claim number
- The number of approved sessions provided by your insurer
- Contact details for your relevant insurer and case manager
- If available, a copy of your job description for your pre-injury role
- Return a signed copy of our service agreement consent form to allow us to speak with your support team
